1. What is Case Management?
It is a co-ordinated programme designed to improve both the quality and the cost effectiveness of healthcare (Sub acute facilities or Nursing Care at home), and identifies and eliminates redundant and overlapping healthcare services.
Case management also ensures that members do not have to be hospitalised longer than necessary.
2. When can I receive this alternative care?
Immediately following an acute incident of trauma, medical illness or surgery.
3. What happens if I need care after being hospitalised?
This is subject to the Case management process prior to discharge from hospital and will only be authorised provided you still need to be in hospital.
4. What happens if I need frail care?
This is unfortunately not funded by the Medical Scheme.
